News & Announcements
Named GP for all patients
All patients now have a Named GP. This GP will have overall responsbility for the care and the support offered via our surgery. You can still, of course, book appointments to see any of our GPs. Either select the appropriate name when booking on-line or let our Receptionist know your preference when booking by telephone or in person. We hope that the continuity of seeing a named GP will be of benefit to you.
View Your Medication, Immunisations and Allergies On-Line
If you have a Patient Access (On-Line Appointments and Repeat Prescription Service) log-in, you can now view your medication, allergies and immunisations that we have recorded for you on our clinical systems. To view this information all you need to do it log-in as if you were making an appointment; you will then see a new section marked ‘Medical Record’. Just click on ‘View Your Medical Record’ and the details will show. Please note that this information can only be viewed via your personal Patient Access log-in. If you don’t yet have a log in, just follow the link at the top of our home page and request a log in or contact reception.
The 1 1 1 Service
If you feel you need urgent medical advice during the day you can now call the 111 number. This is also the number you dial when our surgery is closed to access the Out-of-Hours service. Simply dial 111 on any phone and you will be connected to a skilled advisor who will be able to assess how best to deal with your query.
Call 111 when it’s less urgent than 999
Care.Data/HSCIC/SCR – How information from your medical record will be used in the near future
Care.data/HSCIC – Confidential information from your medical records can be used by the NHS to improve the services offered to provide the best possible care for everyone. This information along with your postcode and NHS number but not your name, will start being extracted to a secure system where it can be linked with other health information. This allows those planning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you. Your data may also be made available to organisations outside of the NHS, such as universities and commercial organisations. We have no choice but to allow the HSCIC to extract the information. The Government has had a leaflet regarding this data upload delivered to every household in the UK.
You will be able to read more about this on the NHS England web site, which carries full details. It can be found by clicking on this link: www.nhs.uk/caredata
You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or wish to prevent this from happening, please contact the surgery and ask for an Opt-Out Form.
Alternatively please click here for an information leaflet and Opt-Out Form.
Summary Care Record – Your data has been uploaded to the SCR to enable other hospitals and healthcare providers to view your records. You can find further information by clicking here.
Again you have a choice and can download an Opt-Out form for the Summary Care Record here.
Note that these are two different data uploads so you will need to complete both forms if you wish to opt out of both data uploads. Please ask us if you wish for further information or clarification.
