Advice for those Travelling Abroad

We have a skilled team of nurses who can give you advice on the types of vaccinations you need when travelling. The type you need will vary depending on where you are going and what you are doing once you are there.
A travel vaccination request form is required for each traveller and should be completed and returned to the surgery well in advance of your trip as some vaccinations require a course of injections. These forms can be completed on line by clicking on the link at the top of this page or you can print out the form found the ‘Forms’ section to the left of this page.
You will need to provide us with details such as travel plans, dates, length of stay and countries to be visited. Upon receipt of the form an assessment will be completed by one of our Practice Nurses who will then contact you by telephone to advise the recommended vaccines and any associated fees.
Vaccinations cannot be given without a completed form for each traveller. After your appointment you will be given a record card for you to keep, letting you know when and if you will require further vaccinations.
IMPORTANT INFORMATION
Please note that some travel vaccinations and all anti-malarial medication are not available on the NHS and will incur a charge. The Practice Nurse will advise of any applicable fees.
Payment can be made using cash or cheque. We do not accept credit or debit cards.
Patients under 16 years of age require a parent’s consent signature.
